# Peptide Empire has expansion plans

> Peptide Empire follows expanding peptide ambitions: semaglutide’s kidney results, immune trials, growth hormones, desire and the reach of longevity claims.

**A journal of peptide ambitions**

Blood sugar, body weight, kidneys. A molecule can acquire a much bigger job description. Every added ambition has to earn a place.

## Semaglutide reaches the kidney trial

The kidney paper is where the scale of the ambition becomes clear. In [FLOW, a trial of 3,533 people with type 2 diabetes and chronic kidney disease](https://pubmed.ncbi.nlm.nih.gov/38785209/), semaglutide reduced a combined outcome that included kidney failure, at least a 50% fall in estimated kidney filtration, or death from kidney or cardiovascular causes. A drug associated with appetite and blood sugar had been tested against the course of a serious organ disease.

Those kidney findings earn my respect. FLOW concerned people with diabetes and established kidney disease. Extending the result to a healthy person’s future kidneys would require a different trial.

Peptide Empire takes its name from this restless accumulation of ambitions. Weight, immunity, growth, sex, aging: the territory gets larger very quickly. Some of the expansion has substantial trials behind it. Elsewhere a hormone reading or an account of better sleep is doing an extraordinary amount of work.

## Appetite trials and a weight-loss comparison

Semaglutide is an especially useful place to begin because several of its ambitions were tested separately. [STEP 1 measured weight change in adults with overweight or obesity without diabetes](https://pubmed.ncbi.nlm.nih.gov/33567185/). [SELECT followed people with established cardiovascular disease and overweight or obesity, also without diabetes](https://pubmed.ncbi.nlm.nih.gov/37952131/). FLOW addressed a different population again. The programme becomes more impressive as those distinctions become visible.

It also becomes less tidy. A [direct weight-loss comparison favoured tirzepatide over semaglutide](https://pubmed.ncbi.nlm.nih.gov/40353578/). I owe no allegiance to a reigning champion: one drug can lose that comparison while retaining useful findings about other outcomes. The [semaglutide essay](/semaglutide) follows those separate contests, including their side effects.

## The other ambitions

Thymosin Alpha-1 has a long immune-research history, but its largest sepsis trial [found no statistically significant reduction in deaths at 28 days](https://pubmed.ncbi.nlm.nih.gov/39814420/). The [thymosin page](/thymosin-alpha-1) starts with the earlier trial and follows the survival question into the larger test.

For CJC-1295, the striking achievement was chemical: a blood-protein attachment prolonged its action. [Growth hormone and IGF-1 stayed elevated in healthy adults](https://pubmed.ncbi.nlm.nih.gov/16352683/). Sleep, strength and recovery remain much larger claims than that result can carry. [CJC-1295 and its albumin attachment](/cjc-1295) gets into the distinction between DAC and no-DAC.

PT-141 reached a defined prescription use as bremelanotide. Its [phase 3 trials measured desire and the distress associated with low desire](https://pubmed.ncbi.nlm.nih.gov/31599840/); the [PT-141 page](/pt-141) gives those outcomes room of their own.

Then there is NAD+, a coenzyme rather than a peptide. It has acquired neighbouring territory in the longevity discussion. The [human evidence for broad benefits remains limited](https://pubmed.ncbi.nlm.nih.gov/41083806/), even though blood levels can rise. [NAD+ precursors: NMN, NR](/nad) follows the narrower muscle and walking findings. NAD+’s walking and muscle findings interest me; rejuvenation is a harder claim to pin down.

## A catalogue of the claims

The [comparison page](/compare) puts the ambitions alongside the actual outcomes. The [reading list](/references) contains the published sources, and [about Peptide Empire](/about) explains why this field deserves such close attention. A large name leaves plenty of room for modest conclusions.
